Please register by completing a form at reception.

New patients are provided with a welcome pack.

You will be asked to complete a brief questionnaire about your medical background.

You will also be invited to attend for an initial appointment with a nurse for a health check.

If necessary you may need to speak with or see one of the doctors about ongoing medications.

Please ensure that you have 1-2 weeks supply of your medication from your last practice before registering with us.

How we work

At Earby surgery the doctors work as a team. You will register with the practice, rather than with a specific doctor.
Many patients prefer to get to know one doctor, and our system allows that. However it also allows you to see another doctor in the practice e.g. a male/female doctor, or a doctor with a specific expertise.

As doctors we prefer that where possible you see the same GP for an episode of illness. We find that this leads to higher quality of care. It saves you repeatedly explaining your problem to different doctors, and reduces the risk that the 'thread' of the problem is lost and fragmented. As a doctor it can sometimes be hard to grasp the true nature of a half-assessed problem; rather like starting to read a book from half way through.
